Hi Karin, I am sorry I missed this comment previously.
Acuity scores are good..but I've yet to see an ideal one! :) It is tough, especially when you have an assignment and the responsibilities that go along with being charge. In the ED, its more organization and patient flow; on the floors it seems more geared towards making the schedule and keeping up on staffing, although I am sure my experience on the two floors I've worked on is certainly not comprehensive!
Its never perfect, and someone always complains...just do the best you can and you will do fine!

Thanks for sharing Karin.
It's not your fault that she died, and its not your fault that you didn't know she was going to die. Nothing you did or didn't do would have changed the outcome--just like my experience with Mr. B. I think sometimes we are handed these experiences in order to learn for next time.

Wow, that experience, you will remember forever. Thanks for sharing.

I have learned to pay attention when my patient says she is dying. A couple of years ago, a patient, a new admit, told me she was dying. For the lack of the luxury time and because I had 5 other, sicker patients that night, I did not get a chance to really sit with and talk with her. She died during my shift, unexpected, yes with vitals signs stable and everything--until the tele monitor read severe brady then asystole within seconds! She had a DNR status so we did not do all the measures to revive her. Nobody was putting a blame on me. But somehow, I had this guilt feeling that I should have sat with her, for at least 5 minutes just to talk. The chaplain sensed this and gave me a hug. I broke out into tears. I should had called the chaplain, at least, to come talk with her before it was too late.

Great job! You are brave!

Sometime, this year, I will do it (charge). I keep postponing it for the fear of handling the assignments. There is always a complaint from one or 2 nurses. When my unit nails down an ideal acuity scoring, I will volunteer to train as charge.

All the time I hear there is a shortage of nurses. My Granddaughter is a LPN and she paid to take the test to go to school to become a RN. She felt she did goo d on the test but they say she missed it by 2 points now this is just to get in the school. I asked could she see her answers and they say NO. Now she can only take this test to become a RN 2 times in her lifetime and she has to wait a year to take it again. Is this sensible. NO wonder there is a shortage. She is very passoniate about being a nurse so what gives. Do they not want any more nurses to keep the salary high. Now I know they imprt nurses from other countries to come here why not let our people that has the same culture have this opportunity. Would like to hear from other people.
