I was still new to the ED as a RN less than a year. I am a big advocate of the dying patient and dying with dignity. A 40 something yr old man came in the ED with SOB and “choking” per the family. He was hospice and the nurses did not educate the family of what they may encounter moments before thier loved one left them. The family described a choking gasping episode which we could only think was the begining of the end. He came to me with a pressure of 70/S and he was very much with it and staed ” I want to die in my bed at home ” I looked at the ER MD and the family and I knew I had to get this guy home quickly . We were together about 2 hours and I stayed by his bedside throughout the entire time. I didnt know how or if it was in Gods plan but I wanted this guy to be strong enough to make it home. Of course he was in pain and quite anxious. I pumped him up with Normal Saline got his pressure over 100/S gave him a touch of Morphine calmed him down. I called our ambulance service and told the EMTs he can not die in the rig…he must die in his home in his bed with his family around him I didnt want them to treat him just like any patient…this man was special ….he was going to die and his last wish must be made. I taught his family what they may see as he passes..and that some times its not too pretty they thanked me . All I could do is pray he got to his home. The EMTs returned he said goodbye and kissed his wife and his daughter as he was layed in his bed ..it was not ugly it was not painfull. his wish was made I never cry .Im not kinda woman…I cried with happiness for his soul was content . If I ever do anything in my carreer nothing will compare to that.
